Get specialist nursing care in the community for children aged 0 to 18 with complex medical needs. We work closely with your family and other professionals.
At 18, your child's care moves to the District Nursing Team.
Specialist community nursing for children and young people (0-18) with complex medical and nursing needs, working in partnership with the family and other professionals.
Physical or mobility Health and wellbeing Home visit Calderdale
Manages complex nursing care in the community, using a supported, holistic approach. At 18, care transfers to the District Nursing Team.
