Specialist social work support for children with severe or complex disabilities, up to age 18.

The Children's Complex Health or Disabilities Team (CCHDT) provides specialist social work support for disabled children and young people (0-18) whose needs are severe, profound or complex and have a long-term, substantial effect on the child and family. Four locality teams cover the Bradford district.
